I understand that if I am hired and have provided false or misleading statements or omissions of fact on this application or any other credit union document shall be considered sufficient grounds for separation of employment. I authorize investigation of all statements contained in this application for employment as may be necessary in arriving at an employment decision. Specifically, I understand that the credit union will obtain a consumer credit report as part of processing my application. I also understand that the credit union will obtain a criminal background review as part of the processing of my application. This consent will not affect my ability to question or dispute the accuracy of any information contained in my report. I acknowledge that I will be asked to sign additional consents for this investigative information. This application for employment shall be considered active for a period of time not to exceed 3 months. Any applicant wishing to be considered for employment beyond this time-period should inquire as to whether or not applications are being accepted at that time.
